Navigating recovery from a serious mental illness (SMI) can be a steep climb. Many members receiving SMI services through Mercy Maricopa Integrated Care are unaware that they are eligible for another tier of services in addition to those provided at their clinics. The Peer & Family Referral Center (PFRC) can educate eligible members about available services and open up a new world for them.
After receiving a complete referral packet, a PFRC Navigator reaches out to the member to begin the Peer Services journey. They will meet for intake to discuss strengths, needs and goals, as well as all the choices on their recovery path.
The member selects agencies and the PFRC Navigator arranges tour, which they take together. Then, the Navigator supports the member through intake and coordinates with the member’s clinical team to keep them updated on progress.
The member is empowered to select where they wish to be picked up. The PFRC provides transportation for the member throughout the placement process, including tours, intakes and return trips.
