The Peer & Family Referral Center (PFRC) is a community collaboration that supports member voice and choice. This program partners with members to discover their specific needs, working closely with other community agencies to provide resources and services that best meet those needs. This highly collaborative program works with each member’s support team to break down service barriers and ensure that the member and their families get the services and resources they need in a timely manner. Since opening in 2015, the PFRC has placed over 300 members in peer and family services.
Navigating recovery from a Serious Mental Illness (SMI) can be a steep climb. Many members receiving SMI services through Mercy Care are unaware that they are eligible for another tier of services in addition to those provided at their clinics. The PFRC can educate eligible members about available services and resources. Opening up a new world for them.

A peer-driven model centers on support from individuals with lived experience. By offering free services (for those who are eligible), we simplify navigation, streamline referrals, and provide a single, trusted hub for information, resources, and peer support. This approach promotes equity, reduces confusion, and helps people connect to the support they need more quickly.
